USD #113 Enrollment Information for 2010/11

ENROLLMENT OF STUDENTS IN PRAIRIE HILLS USD 113

Classes will begin on Thursday, August 19, for all students in Prairie Hills Unified School District No. 113.  Teachers will have two days of orientation, August 16 and 17 and a work day on August 18 prior to the beginning of classes.

Enrollment and fee payment for all students attending Prairie Hills Unified School District No. 113 will take place August 3rd from 9 a.m. - 4 p.m., August 4th from 12-7 p.m. and August 5th from 10 a.m. – 4 p.m.  Students who pre-enrolled last spring should check and verify classes and pay the necessary fees.  Any students who cannot be present at enrollment time should call the principal’s office at the building in which he/she will attend classes.  (Telephone numbers are as follows: Wetmore Attendance Center, 785-866-2860; Sabetha Elementary School, 785-284-3448; Sabetha Middle School, 785-284-2151; Sabetha High School, 785-284-2155, Bern Elementary School, 785-336-2293, Bern High School, 785-336-3031, Axtell High School, 785-736-2237, Summerfield Elementary School, 785-244-6221)

Entrance requirements for new students are as follows: transcript from accredited schools attended previously; if entering school in Kansas for the first time, the students must furnish a copy of birth certificate, immunization records and a health assessment (as below) if under eight-years-of-age.  If entering kindergarten, children must be 5 years of age by August 31.  Kindergarten students must have birth certificate and immunization and a health assessment required by KSA 72-5214.

REQUIREMENTS FOR ADMISSION

1. Pupils entering school in Kansas for the first time (including kindergarten
pupils), as required by Kansas Law:

A. Must present state issued birth certificate.
B. Must furnish health certificate by Kansas doctor.

2. To enter kindergarten the pupil must be five (5) years old by August 31,
2010.

3. To be admitted to the first grade, the pupil must be six (6) years old on or before August 31, 2010, or furnish proof of having completed an authorized kindergarten class.

4. To be admitted to any other grade the pupil must furnish proof of having satisfactorily competed the necessary credits in an accredited school.  A transcript of credits is necessary for students in grades 9 through 12.
