Guidelines for Medication Use at Sabetha-Wetmore USD 441

MEDICATION USE IN SCHOOL

SABETHA ELEMENTARY SCHOOL AND SABETHA MIDDLE SCHOOL

Prescription medications—All prescription meds must be kept in the nurse’s office or in the school office for the middle school.  They should not be kept in the classroom, book bags, or lockers.

All prescription meds must have a ”Permission for Medication” form signed by both the parent and the prescribing doctor on file in the nurse’s office.  This form can be obtained from the school office or from the nurse.

All prescription meds must be in an original container with the student’s name, drug name, dose and frequency clearly marked on the label.

The only prescription drugs that will be given at school are meds that need to be given at school.  If they are ordered 3 times a day, they can be given before school, after school and at bedtime.  Meds ordered 4 times a day may need to be given at school.

Certain daily meds needed for school success, may be given at school with the prior approval of the principal.

Rescue inhalers may be carried by the student with permission of the parent and doctor on file in the nurse’s office.

Over-the-Counter (OTC) medications

SES—All OTC meds must be kept in the nurse’s office and not in the classroom.  The only exception is for cough drops, which can be kept in the classroom if accompanied by a note from the parent.

All OTC meds must have a “Permission for Medication” form signed by the parent and on file in the nurse’s office.  We need a new form signed each year.

If your child requires OTC meds on a regular basis (more than once or twice per year) you are asked to provide a small supply for your child to use at school.  This will be kept in the nurse’s office and labeled for your child only.

SMS—All OTC meds will be kept in the school office and not in book bags or lockers.  The only exception is for cough drops, which can be kept with the student if accompanied by a note from the parent.

If your child requires OTC meds on a fairly regular basis, you are asked to provide a small supply for your child to use at school.  This will be kept locked in the school office and labeled for your child only.

SABETHA HIGH SCHOOL

Prescription medications-- All prescription meds must be kept in the school office and not in lockers, book bags, or purses.  Unless a prescription med needs to be taken prior to arriving home, they should not be brought to school at all.

All prescription meds must have a “Permission for Medication” form signed by both the parent and prescribing physician on file in the nurse’s office.  This form can be obtained from the nurse’s office or school office.  The prescription med must be in a pharmacy container with the correct pharmacy label intact.  The pharmacy will give you an extra one for school use if you ask them.

OVER-THE-COUNTER MEDICATIONS (OTC)—All OTC meds such as Tylenol, Motrin, Advil, Midol, Tums, Aleve, etc may be carried by the student IF they have a note from the parent with the name of the allowed med and the date.  Under no circumstances is the student allowed to share or give another student any meds, OTC or prescribed.  Failing to follow this guideline may result in disciplinary action by the school.

WETMORE ACADEMIC CENTER

Prescription and Over-the-Counter medications—All medications are to be kept in the school office—including over-the-counter (OTC) meds.  If your student needs meds for the short-term, you may send the medication, in it’s original container with a note signed and dated by you, and your student may come to the office to receive their dose.  If your student needs a prescription medication we will also need the “Permission for Medication” form signed by the parent and the prescribing doctor on file in the nurses office.  You may obtain this form from the school nurse or the school office.

No medications (prescription or OTC) are to be kept in lockers, book bags, or purses.

Any and all exceptions must be approved by the Principal.
